Restoration of Registration
You may restore your registration only:
- if you had current registration during the period 1 July 2007 to 30 June 2008;
- you wish to remain currently registered for the period 1 July 2008 to 30 June 2009.
The 2008-2009 restoration of registration form is available below.
If you do not meet both of the above requirements you cannot use the 2008-2009 restoration form. You will need to make a new registration application and should go to the appropriate form on this site.
- You can type MOST information into the 2008 restoration of registration forms
- You must PRINT the completed form prior to signing it
- You must RETURN the printed and completed form to the Board by either
- mail at locked Bag 1341, Brisbane QLD 4001; or
- fax on number 3225 2514 (remember to fax all sides of the restoration form); or
- in person at 8th Floor, Forestry House, 160 Mary St, Brisbane QLD 4000
