Change of Address (postal or email)
Pharmacists can change their contact address (postal or email) by telephone, mail, fax or email. Please be advised that if you wish to change your address by telephone, you will be required to answer some security questions to ensure that the office is updating the correct records.
Change of Name
Pharmacists wishing to change their name on the Register are required to provide the Board with a certified copy of the relevant documentation, e.g. Marriage Certificate or Deed Poll.
